Forms
	Medical Release Form	Must be filled out by all parents and turned into the Manager or Coach of the team. It is a requirement that all Managers/Coaches keep a copy with them during all games, practices or Little League functions, in which the children are participating.
	Volunteer Form	Required for all Volunteers. It is a Little League mandate that any adult, who wants to be on the field with the kids, or on the Board of Directors, fill out a volunteer form and submit it to the leagues Safety Officer. No adult is allowed to work with the children until the form is turned in.
	Sponsor Form	The DMLL Sponsorship Form. Please complete and send your requests to sponsors@dmll.org
	Academic All-American Program Application	The DMLL Academic All-American Program recognizes DMLL participants that also excel in the classroom. Please complete the application form for consideration in the program.
	Registration Forms	Registration Day instructions and all the necessary forms to complete the registration process.
	Regultion V(a) 12 Year Old Waiver Form	Wavier form used to request a 12 year old to play down in the Minors divisions.
