 | Medical Release Form | Must be filled out by all parents and turned into the Manager or Coach of the team. It is a requirement that all Managers/Coaches keep a copy with them during all games, practices or Little League functions, in which the children are participating. |
| Volunteer Form | It is a Little League mandate that any adult, who wants to be on the field with the kids, or on the Board of Directors, fill out a volunteer form and submit it to the leagues Safety Officer. No adult is allowed to work with the children until the form is turned in. |
